After an AP investigative report found a striking pattern of inconsistent reports and often light penalties at bases in Japan, officials put pressure on the Pentagon to release records.
While teaching militant recruits how to make a car bomb on Monday, an instructor accidentally set off explosives during the demonstration. The blast killed 21 and alerted authorities of the existence of the training camp north of Baghdad.
